3. Participation does not guarantee any diagnosis, treatment, medication, or clinical outcome.
4. The patient is responsible for providing accurate and complete medical information.
5. Confidentiality & Use of Data: The organizers may use the information provided solely for medical records, follow-up consultations, patient education, and healthcare communication while maintaining confidentiality in accordance with applicable laws.
6. Doctor's Discretion: Medical advice and prescriptions, if any, shall be solely at the discretion of the consulting doctor.
7. Modification & Cancellation: The organizers reserve the right to modify, postpone, or cancel the camp without prior notice due to unavoidable circumstances.
8. Consent to Terms: By submitting this registration form, the participant agrees to these Terms & Conditions.
9. Jurisdiction: Any dispute arising from this program shall be subject to the jurisdiction of Ahmedabad, Gujarat, India.
